New Patient Registration
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by bringing along your NHS medical card or by completing forms available below. Each person will need a registration form and a questionnaire. There is a separate questionnaire for under 16's. The forms are available to download from the above "Registering Online" tab.
You will also need to provide photographic proof of ID and proof of residency such as a recent utility bill.
We will make a note on your records if you prefer to see a particular doctor or nurse.
We recommend that new patients undertake a health check with a practice nurse.
Medical treatment is available from the date of registration which will only be completed upon receipt of the required identification & proof of residency. Please contact reception for further information.

Military Veterans
A veteran is someone who has served in the armed forces for at least 24hours. There are around 4.6 million veterans in the UK.
When Service men and women leave the armed forced their healthcare is the responsibility of the NHS
All veterans are entitled to priority access to NHS hospital care for any condition as long as it is related to their service, regardless of whether or not they receive a war pension
If the health speciality or service you are dealing with is unaware of your entitlement for priority treatment, you are actively encourage to tell them about it and ensure they know that 'you have served'. Failing that, you can enlist the support of your local armed forces network, your local authority Community Covenant Lead or one of the national service organisations such as the Royal British legion
All people leaving the armed forces are given a summary of their medical records which they are advised to give to their new GP when they register. If you no longer have a copy of the summary you can apply for one via the 'Requests for personal data and service records' page on http://www.gov.uk
Veterans are encouraged to tell their GP about their veteran status in order to benefit from priority treatment. If you are moving to a new location in England or returning from a posting abroad you need to make sure you register with an NHS GP. More information can be found on the NHS Choices website at:
